DBHW offers community based mental health and substance abuse services to patients across the life span through one of our outpatient offices or in collaboration with other behavioral health providers.  Our highly skilled physicians and clinicians provide quality, compassionate care for our patients by offering consultation, medication management and therapy services.  With multiple offices and clinicians, all referrals are centrally received through our patient services coordinator.  Outpatient mental health services are offered in Warren, Bradford and Corry, PA.  Outpatient substance abuse services are offered in Warren, Marienville, Tionesta and Titusville, PA.

Developmental and Intellectual Disabilities Program provides services to individuals from age 3 to death who have significantly sub-average general intellectual functioning (IQ of 70 or below) that is accompanied by significant limitations in adaptive functioning in the following areas: communication, self-care, home living, social/interpersonal skills, use of community resources, self direction, functional skills, work, leisure, health and safety.  Case managment activities may include: Outreach, referral, assessment, coordination of service, planning, monitoring and client advocacy. Services may include Home and Community Based services such as habilitation and respite, Employment services such as Pre- vocational or Job support and Community residential services. Services may be based on need and available funding. Current Waiver programs include Person/Family Directed Support Waiver (PFDS) and the Consolidated Waiver for persons who qualify for an ICF/MR Level of Care.
