Eligibility

If you are going to require financial assistance with services, there are typically some eligibility criteria that need to be met. Various programs will have income eligibility requirements and many will have age or other requirements as well. The specific eligibility requirements for each individual program are listed below, but to insure the information is the most current you should contact the individuals included in the listing; or any of the LINK Partners.

Level of Care Assessment:

In order to insure people receive the most appropriate services possible, many of the programs also require that an assessment of level of care be conducted.

Experience Inc. (the Warren-Forest Counties Area Agency on Aging) through it’s Care Management Department provides access to a variety of programs to enhance the quality of life for Pennsylvania citizens, generally age 60 and older, who desire to remain independent for as long as possible. From the time of the initial telephone call to our Intake and Referral Specialist you will gain information on a wide-range of programs available to you.

Experience Inc. is the State contracted agency responsible for administering the level of care assessment and determining the level of care that is needed in order to provide appropriate services. Experience Inc. must also conduct Level of Care Assessments for all individuals, regardless of the individuals age, who need to access Medicaid funds in order to assist with the payment of long term care services.

Once the Level of Care Assessment is completed, if an individual is determined to be best served through an Aging Home and Community Based program, a Care Manager will assess the individuals needs and work with the individual to establish a care plan in order to meet these needs. The assessment will be reviewed by management staff as well as a clinical review by nursing staff and a consultant physician when appropriate. For those services that are care managed by the agency, the staff also develop an on going relationship with the consumer through home visits, telephone calls and communication with the individuals physicians and outside agencies that are also providing other services. The Care Manager will also coordinate services with outside providers who are trained to service the individuals needs that are addressed in the care plan. The Care Manager maintains an on going relationship with the consumer through home visits, phone calls, communication with the providers and by continually assessing additional needs and coordinating services to fulfill the needs of the individual.

The Warren County Assistance Office determines eligibility for Cash Assistance programs, Medical Assistance programs (including Long Term Care Nursing Home and Home Community Based Services), SNAP (Supplemental Nutrition Assistance program formerly known as Food Stamps), and LIHEAP (Low Income Home Energy Assistance Program).
